Includes One acupuncture/dry needling per month. Renews automatically each month for just $75 under the Bare Bones Membership. Each month the membership includes 1 needling session that can be used at any location.

PLEASE READ, AND YOU WILL SIGN AN E-SIGNATURE TO AGREE TO EACH LINE INDICATING THAT YOU HAVE READ AND AGREE TO THE FOLLOWING MEMBERSHIP PLAN AND ITS TERMS.

You acknowledge receiving and reading a completed copy of this agreement before signing and are of sound mind. You understand that the rules, regulations, and terms of your Hokey Pokey are incorporated into this agreement.

This agreement is being entered into today's date so that the client can purchase and receive no less than (1) Acupuncture/Dry Needling each month upon successful payment of $75 on the client's scheduled billing date between a hip joint, LLC, and you, the client. We will be adding an online fee of 3% per online transaction.

The client's scheduled billing date will occur the day this contract is agreed upon and will continue on the same date of each calendar month. The client will be billed each month until this agreement is terminated or if payment has been unsuccessful for over 30 days.

The client will be able to receive the following services at a discounted rate with an active membership;

1-Hour Massage $45, Acupuncture Session $65, Cupping $20, Additional Chiropractic Adjustments $35.

The price per service or other discounted services cannot be combined with any other discounts, coupons, or specials. If you miss a service for the month (without previously discussing with the office) you will be charged as normal and the service will NOT be carried over to the next month or anytime in the future. You may place your membership on hold at any time by contacting our offices. At that time, you will not be charged your membership fee or receive services.

If you would like to cancel your Hokey Pokey membership contract, please contact our offices IN WRITING. If you fail to contact our offices with at least 7 business days notice of your normal billing date and receive confirmation of your membership being cancelled, your membership will not cancel until the following month. Payments due prior to the effective date of cancellation will be charged as scheduled. Upon termination or cancellation of membership, all unredeemed adjustments will expire.

At any time, 'a hip joint' has the right to cancel your membership for any reason. If 'a hip joint' should cancel your contract, you will NOT be reimbursed for any received services. A Hip Joint has the right to adjust prices of services based on market trends, inflation, and demand. We will give a 30-day notice for any price increase.

By signing below, I authorize 'a hip joint' to charge to the account I have specified. Monthly dues and/or renewal fees will be withdrawn on or after the same day of each month. I understand 'a hip joint' may continue to charge my account or cancel my membership in accordance with the terms and conditions of this agreement. Additionally, I authorize 'a hip joint' to charge my account on file in lieu of presenting it for any services received, at my request. We agree to sell, and you agree to purchase the membership, goods, and services herein, you agree to pay us for the membership, goods and services according to the payment schedule below. Your signature below indicated your agreement. All persons signing this agreement are equally responsible for paying it in full.
